PDAA Impact Questionnaire

At the last Plymouth Dementia Action Alliance meeting, an agreement was made to measure PDAA’s impact on those living with Dementia and their carers.

After much discussion with members of the PDAA, a questionnaire has been made to fulfil this aim. To complete the questionnaire, please click here. To meet General Data Protection Regulations 2018, this questionnaire will be anonymised.
